When appointing as a new patient
with our clinic, call our office and our receptionist,
Rachel, will offer times available for the practitioner
you request. If you are interested in seeing Linda
S. Berry, DC or
Kathleen Dvorak, DC, the first new patient appointment
will be an hour in duration. The fees for the first
visit are $65.00 for the examination and $55.00 for
the initial treatment. Standard office visits thereafter
are typically 15 minutes long and are $55.00. If you
have any recent lab results or x-rays that may be relevant
to your visit, please bring them with you. Visits are
payable by cash, check or credit card.
If you'd like to make an appointment
with Renée
Rutz, Lisa
Fox-DelZoppo or Kirsten
Mowrey, that appointment will be an hour long and
the fee is $75.00 payable by cash or check. New patient
paperwork for both chiropractors and massage therapists
are available to download. See "New Patient Forms"
in the column to the right.
Chiropractic coverage is provided
under many insurance policies including BCBS, Medicare,
Aetna, Messa and many others. Each policy varies according
to the plan your employer has chosen or what is available
for you to choose from. We do bill all insurance companies
as a courtesy to our patients. However, we do not participate
with any insurance plans as "in network" providers.
This may mean differences in reimbursement for services
for some insurance plans. Massage therapy may also be
a covered benefit under some insurance plans. Typically,
the patient must be under the care of a chiropractor,
osteopath or MD who has prescribed the massage therapy
care.
Our Financial Coordinator, Tonia,
verifies each chiropractic new patient's insurance information
and will explain your particular coverage.
There is a 24-hour cancellation
policy. If your appointment is missed you will be charged
the full amount of that service. However, if you miss
an appointment due to illness or an emergency there
will be no charge.
If seeing a chiropractor,
we request you wear or bring loose fitting clothing
such as warm up pants, shorts, T-shirts, etc., as your
visit will involve structural examination and treatment.
If you are seeing a massage
therapist the best way to receive a massage or other
bodywork is to be unclothed. Our practitioners suggest
you undress to the level of your comfort and you will
be appropriately draped at all times.
Gift certificates are available
for all services at CCBT. Contact our office and we'll
be happy to make arrangements for purchase.
